A Endodontia Guiada uses imaging data and digital planning to help define a previously studied trajectory to the region of interest. The procedure no longer begins in the chair — it begins on the screen.

Starting from the patient’s tomografia, you define in software where to enter, at what angulation and how far to go. That plan becomes a guide — the Endoguide — which guides the bur along the studied trajectory. It is the approach most described in the literature for canais calcificados, and the same logic extends to post removal and endodontic microsurgery.

Definitions

Endoguide, answered directly.

Endoguide is the application of Guided Endodontics in which a digitally produced guide directs access to the root canal system, based on virtual planning performed on three-dimensional imaging.

The trajectory is defined in software before the bur touches the tooth, and the guide rests on the adjacent teeth to reproduce it.

Guided endodontics is the approach that combines cone beam computed tomography, intraoral scanning or surface digitisation, virtual planning and a digitally produced guide to direct endodontic access in selected cases.

Described in the literature since 2016. Also referred to as guided endodontic access or, in its static form, static guided endodontics.

A calcified canal is a root canal that has undergone progressive deposition of mineralised tissue, narrowing or obliterating its lumen — a condition also described as pulp canal obliteration.

When the canal cannot be located through the usual landmarks, exploratory access increases structure loss and the risk of deviation. That is where the guided approach has been most studied.

In conventional access, the trajectory is defined during the procedure, based on visible anatomy and operator experience. In guided access, it is defined beforehand, in a digital environment, and reproduced chairside through the guide.

The difference lies in the stage at which the decision is made, not in any promised outcome.

Cone beam computed tomography provides the internal anatomy of the root and canal; intraoral scanning or surface digitisation provides the external reference on which the guide will rest. Planning arises from superimposing the two datasets.

Neither needs to be yours. The scan can be taken at a radiology clinic, and one lesson covers the referral protocol.

No. Guide fabrication is a stage that can be carried out by specialised planning and production centres, without the clinician having to buy a printer or set up an in-house laboratory to get started.

The same applies to planning. What cannot be outsourced is reading the case — and that is what the course teaches.
